2012 Health Halo Award

 

You know how hard you and your staff have worked to make menus lighter, brighter and more kid-happy! Now tell us how you put a Health Halo on your nutrition program.

To enter the 2012 Little Blue Dynamo Health Halo Award Contest, simply tell us about your initiative and support your narrative with samples of menus, photos, and any other illustrations of ways your department has made “healthy” improvements to your menus and helped get kids excited about health and nutrition. You can mail or email your entry. Click the “questionnaire” link below for more information.

Grand prize is an all-expenses-paid trip for two to the ANC in Denver. The prize includes round-trip travel, hotel accommodations and conference registration. Click the “rules” link below for details.The deadline to enter is April 16, 2012.

Here are some examples of what your entry might include:

  • New fruit and vegetable items added to your menus
  • Healthful recipes/menu items that your kids love
  • Special nutrition- and health-focused activities or programs that your department initiated or collaborated in that may include parents, the community, local farmers, chefs, or health professionals
  • Before- or after-school nutrition programs or activities your department has conducted to help ensure that all children have access to healthful food and/or information

How to present your materials?

  • Fill out the attached questionnaire.
  • Put your support materials in a scrapbook — electronic or hard copy, make a Powerpoint or a video, send us to a website or blog — any way you choose to show and describe what your department has done to get that health halo glowing brighter and involve kids in making better food choices.
  • Please note that more is not always better. Your entry will not be judged on length nor on how sophisticated the presentation is. We are looking for your innovative and creative ideas- successful approaches that worked in your school.

The program(s) you describe in your entry must have taken place no earlier than August 1, 2010. You may send your entry any time between Monday, February 13, 2012, and Monday, April 16, 2012. Entries received after the deadline will not be considered. The winner will be announced on or before May 15, 2012. Entries, including entrants’ names, schools’ names, descriptions of the programs, photos of entrants, and photos entered to the contest may be used in promotion, including press releases. CONTEST DATES

Entries for the will be accepted beginning Monday February 13, 2012 through Monday, April 16, 2012 (“entry period”). All entries must be submitted and received within this time period. Any entries not received within this time period will not be eligible and such entries will not be considered.

ELIGIBILITY

Entrant must be currently employed in the foodservice department of a public or private K-12 school in the United States. Current employees of the U.S. Highbush Blueberry Council, its sponsors, subsidiaries, affiliates, owners, administrators, and their respective members, officers, directors and shareholders (collectively “U.S. Highbush Blueberry Council” or “Sponsor”), Lewis & Neale and CRT/tanaka and any such employees’ immediate family living in the same household are not eligible to enter or win the Contest.

FINALISTS

Winner will be notified by phone. They will be required to execute and return an affidavit of eligibility, authenticity, liability/publicity release and assignment of rights within 14 days of notification or the prize will be forfeited and an alternate winner selected.

All entrants acknowledge and agree that all material entered becomes the property of the U.S. Highbush Blueberry Council. The USHBC reserves the right to publish the winner’s name, school affiliation, details about the school’s foodservice program, and material entered for public relations and promotional purposes in any media without compensation. By entering the contest, you agree to the use of your name and your school’s name for promotional purposes without compensation. Failure to comply will result in the selection of an alternate winner.

PRIZES

One (1) school child nutrition department will be awarded all-expenses-paid trips for two (2) to the School Nutrition Association’s Annual Nutrition conference in Denver, Colorado, July 15-18, 2012. Included in expenses: conference registration (value approximately $900); round-trip transportation (value up to $700); hotel accommodations (value approximately $600); incidental expenses (value $100).

The value of each prize may be taxable as income to the winner of the prize, and all federal, state, and local taxes associated with each prize are the sole responsibility of its winner, as applicable.

GENERAL CONDITIONS

The Contest will be governed by these rules. By participating in this Contest, all participants agree to be bound by these rules and the decisions of Sponsor and its agents, whose decisions are final on all matters relating to this Contest. The Contest is void where prohibited by law and it is subject to all federal, state, and local laws and regulations.

Any and all permission needed from school administrators or other authority to enter the contest and to disclose details about the school’s foodservice program is solely the responsibility of the entrant.

Sponsor CRT/tanaka and its officers, directors, agents, and employees are not responsible for and shall not be liable for: (i) lost, misdirected, unintelligible, improperly formatted, damaged, incomplete, or late entries; (ii) any act, failure to act, delay, or error relating to the transmitting or processing of entries; (iii) telecommunications network, phone system, computer system, hardware, or software malfunctions, failure, disconnection, data loss, or other difficulties of any kind, including any injury or damage to a participant’s or other person’s computer related to or resulting from participating in the Contest, or accessing, downloading, or uploading materials relating to the contest; (iv) any conditions caused by events beyond the control of Sponsor that may cause the contest to be disrupted or corrupted; (v) any losses or damages of any kind resulting directly or indirectly from acceptance, use, or misuse of any prize awarded in the Contest or from participation in the Contest; or (vi) any errors in any materials relating to the Contest. Sponsor reserves the right, in its sole discretion, to cancel, modify, terminate, or suspend this Contest without notice if causes beyond Sponsor’s control corrupt the administration, security, or operation of the Contest.

CONTEST VOID WHERE PROHIBITED OR RESTRICTED BY LAW

This contest offer is void where prohibited or restricted by law and is subject to applicable federal, state, local laws and regulations. By participating in this contest, entrants agree to be bound by these rules and by the decisions of the Sponsor, which are final in all matters relating to this contest. Any permission from administrators for contest entry must be obtained by the entrant(s). Prizes are nontransferable. By accepting any prize, winner consents to the use of their name, photo, voice, and likeness (and video, if applicable) for promotional, advertising, publicity or trade purposes in any and all media worldwide in perpetuity by Sponsor without further compensation to winner or any other person depicted in photo unless otherwise prohibited by law. Arrangements for the fulfillment of all prizes will be made by the Sponsor. Receipt of any prize is conditioned upon compliance with any and all applicable federal, state and local laws, rules and regulations.

GOVERNING LAW & VENUE

The New York court (state & federal) shall have sole jurisdiction of any controversies regarding the Contest and the laws of the state of New York shall govern the same. Each entrant waives any and all objections to jurisdiction and hereby irrevocably submits to the venue of those courts within New York County in New York. All claims, actions, proceedings or litigation brought by any party hereto arising under or concerning, referencing, or relating to these Official Rules and/or the subject matter hereof shall be instituted and prosecuted exclusively within the Federal and State courts situated within the City of New York. New York and the parties hereby agree and submit to the exclusive personal jurisdiction and venue of such courts for such purposes.
